The index depends on the measurement of 35 key performance indicators, some of which are related to the performance of the entity in general and some are related to certain services provided by the entity, so two types of forms have been allocated.

Each entity is required to enter a single entity form, in addition to a special form for each service. Example: The Ministry of Health provides three e-services
Number of entity form = 1
Number of service forms = 3
Total forms to be filled = 4 forms

The amount and quality of the data entered into the forms have a direct impact on the evaluation. Therefore, it is preferable to answer as much questions as possible and enter all available information.
